3 Successful Cases Radial approach of PCI to Aberrant RCA: 3 Successful Cases Radial approach of PCI to Aberrant RCA Presenter: Dr Leong Weng San Department of Cardiology, Hospital Sultanah Aminah Johor Bahru Primary Operator: Dr Liew Chee Khoon Secondary Operator: Dr Leong Weng San Introduction: Introduction 1) coronary artery anomalous -1% in coronary catheterization 2) Aberrant RCA known to be associated with SCD, esp if it passed btw aorta and pulmonary trunk. 3) PCI to aberrant RCA is known to be a challenge 1st Case: Mr ABS: 1 st Case: Mr ABS 45 years old gentleman DM, HPT, smoker Presenting with intermittent angina ECG: sinus rhythm Coronary angiogram: Coronary angiogram 2nd case: LHL: 2 nd case: LHL 42 y.o. gentleman Smoker, hyperlipid, FH of CAD Presented with NSTEMI Troponin T: 1.76ng/ml T. Cholesterol: 7.1mmol/L, LDL: 5.0mmol/L Diagnostic Catheterization: Diagnostic Catheterization Failed engagement of RCA with JL4, WRP, AR/AL. Just engaged with MPA: Failed engagement of RCA with JL4, WRP, AR/AL. Just engaged with MPA PCI Transradial: PCI Transradial Final results: Final results 3rd Case-GKH: 3 rd Case-GKH 64y.o lady Hypertension/ DM/ hyperlipid Had mLAD 70% stenosis and aberrant RCA prox RCA lesion 90%, detected since 2004. PCI was done to mLAD Failed previous engagement of RCA via femoral approach Was planned for medical therapy since then. But in 2009, started to have angina Diagnostic catheterization: Diagnostic catheterization Unable engage RCA with : Unable engage RCA with PCI transradial: PCI transradial Summary: Summary Selectively cannulating the anomalous RCA arising from the left sinus of Valsalva is usually challenging owing to the unusual site of its ostium within the sinus and its ellipsoidal shape, the acute opposite direction course of its proximal segment and the lack of specially-designed catheters PowerPoint Presentation: Though femoral PCI has been reported with success and failure, data for successful transradial PCI for left coronary sinus RCA seems still lacking In our cases, it may seems that transradial PCI to such type of RCA yields successful results
